Description
A recorded copy of information supplied for births that were not recorded or were incomplete or incorrect and required by a 1941 PA law. Printed forms provide information for full name and current address of applicant, date and place of birth, sec, whether a twin or plural birth, name of father and mother (with race, age, residence, and occupation at time of birth)), current address of father and mother, information on other siblings at time of birth, name of physician or midwife (if attended birth), affadavits verifying birth from applicant, priest/minister, relative, and non-relative, notarization reference, File number and date, Register number, and date of issuance.
Arrangement: Numerical by filing date.
