"Application for Burial of Deceased Soldier” forms begun with the PA Act of 1885 regarding county obligation to bury indigent soldiers, sailors, or marines. Information includes name of deceased soldier, sailor, or marine; rank, company, and regiment; date of discharge; occupation; date of death; place of burial; and remarks. A “Statement of Account” regarding burial expenses is present with certification information by a notary public and three witnesses.
