Information includes name of applicant (often with township or borough residence), reason for ineligibility (usually medical), signatures of applicant and Justice of the Peace, and date of affidavit. A few medical excuses from military duty for active militia volunteers.
Arrangement: Chronological by affidavit date and then alphabetical by applicant's last name.
